Qualified Intellectual Disabilities Professional (QIDP)

Job Overview
QIDPs are responsible to coordinate and manage supports for individuals at our Community Day Services facility. They work independently and as a team to ensure that the needs of all the individuals are being met and all state and federal guidelines are being followed. All staff working at Self Help Enterprises are encouraging self-advocacy and choices through role modeling, instruction and support to those we provide service to. They work as a team to think creatively to problem solve.

They help create a safe and enriched environment where people with disabilities are free to experience, grow, and have opportunities to gain an increased sense of self worth and purpose.

General Functions
The basic function for a Community Day Services QIDP is to ensure person centered supports and goals both at the facility and in the community. QIDPs work as a team with the Independent Service Coordination (ISC) and Independent Support and Service Advocacy (ISSA) workers to help develop Individual Personal Plans and create Implementation Strategies.

Key Responsibilities
Develop and implement opportunities for Community-Based Inclusion activities based on the interests of individuals served. Develop and enhance community partnerships.
Coordinate, supervise, and monitor staff are engaging in functional, meaningful and person centered activities.
Participate in person centered planning, providing input for discovery tools, attending Personal Plan meetings, and alongside individuals served, develop implementation strategies.
Ensure Implementation strategies are followed and appropriate documentation, including QIDP monthly summaries, are completed in a timely manner.
Ensure assessments and documents (i.e., ICAP, annual paperwork, Human Rights Restriction, etc, ) are completed accurately and in a timely manner.
Maintain current Individual files in accordance with case file organization and retention schedule.
Participate in staff training as assigned and maintain continuing education as required. (12 CEU hours annually)
Liaison effectively with ISSA workers, guardians, family members, residential providers, and other stakeholders in order to provide cohesive supports and advocacy for individuals served.
Respond to medical and behavioral emergencies following appropriate health & safety, crisis prevention, OIG reporting, and CIRAS reporting procedures are followed.
Be familiar with or have a willingness to learn Home Based Waiver Program, which includes some travel and periodic work outside of Self Help's regular business hours.

Minimum Qualifications
Bachelor's degree from accredited college or university in a human services field (including but not limited to psychology, sociology, special education, social work, or rehabilitation counseling).
A minimum of one year experience working with individuals having intellectual/ developmental disabilities
Must meet the qualifications of QIDP. Prefer that you have completed the required training and are on the state registry, but willing to train for the right applicant. Must be able to become CPR/First Aid Certified.
Pass a full background check.

Direct Support Professional (DSP)

At Self Help, the Direct Support Professionals provide supervision, care and engage with a small group of individuals with disabilities. They work independently and as a team to ensure that the needs of all the individuals are being met. DSPs help by teaching skills, engaging in meaningful activities and promoting self-advocacy and choices through role modeling and instruction. They work as a team to think creatively to problem solve. DSPs help create a safe and enriched environment where people with disabilities are free to experience, grow, and have opportunities to gain an increased sense of self worth and purpose.

Qualifications/Requirements
High School diploma or equivalent (GED)
Criminal background clearance
Direct Support Professional (DSP) trained (preferred)
First Aid/CPR trained (provided through agency)
Experience working with persons with Developmental disabilities (2 years preferred)
Able to perform moderate physical tasks
Dependable transportation

Residential Direct Support Professional (DSP)

They work independently and as a team to ensure that the needs of all the individuals are being met. DSPs help by teaching skills, engaging in meaningful activities and promoting self-advocacy and choices through role modeling and instruction. They work as a team to think creatively to problem solve. DSPs help create a safe and enriched environment where people with disabilities are free to experience, grow, and have opportunities to gain an increased sense of self worth and purpose. This position is currently open to fill staffing needs in Self Help's residential programs.
